Your rights

  • Participation in decisions and the opportunity to ask questions.
  • Consent before procedures, except where law permits emergency treatment.
  • Privacy and confidentiality of medical information, subject to lawful disclosures.
  • Access to records and discharge information according to applicable procedure.
  • Information about the identity and role of people involved in your care.
  • A way to raise concerns without affecting appropriate care.

Your responsibilities

  • Provide complete and accurate health, medicine, allergy and insurance information.
  • Ask questions when instructions are unclear.
  • Follow agreed care instructions or explain when you cannot.
  • Treat staff, patients and visitors respectfully.
  • Follow hospital safety, infection-control and visitor policies.
  • Meet financial obligations not covered by an insurer or scheme.
